Appointments

All patients of the surgery who have access to a smart phone, computer or tablet are asked to submit their clinical or medical administrative query via a form on an online facility called ‘Accurx’.  Click here for access to forms or click on the green image above.

How should I book an appointment with a member of the nursing team?

Please note that if you need to book an appointment with a member of the nursing team, you can telephone us as usual (01825 732333) or send us 'admin form' request stating what appointment you need, we will then be in touch with you. If you call before 4.30pm, and there are more than five callers in the queue, you will be given the opportunity for a 'call back' from us meaning you do not need to wait whilst holding on the telephone.

What if I don't have access to a smart phone or computer?

We understand that there will be patients who are unable to submit their request electronically, and you will still be able to telephone us, or visit us in person. In these cases, a receptionist will ask you the same questions as the online form, they will then record down this information. Requests created by our reception team on behalf of patients will then be dealt with in exactly the same way as those who submit their requests electronically.

Patients will not be given an appointment at the time their form is submitted but instead someone from the surgery will make contact once the request has been reviewed.

When will I get a response?

You will be contacted within one working days of submitting a 'health problem' form but obviously urgent cases will be dealt with as a priority as is the case now. It is anticipated that most, if not all, requests, will be reviewed on the day they are received, but please remember if your concerns are very urgent you should ring 111 for advice or go straight to A&E if you are very unwell and require immediate treatment. Any administration requests will be dealt with as soon as possible by the administration team. Requests for routine care will usually be responded to within one working day. 

If all of the 'health problem' forms  for the day have been utilised, a note will be displayed advising that we have reached our safe working limits for the day and therefore patients will be advised to contact 111, as they are now, or, if the patient feels that their query really cannot wait, they are able to call the surgery and talk to a receptionist. You may wish to look again online or call back later to see if the form is available again. As the day progresses more appointments may become available; this situation could arise where not all requests received require an appointment, therefore freeing up capacity.

If a patient uses the admin request function for something which should actually be a medical request, the patient will be asked to resubmit their information using the medical form in order that the request can be triaged in the correct manner.

Do I need to complete a medical form to request a blood test?

For patients who require regular blood test monitoring (and whom we understand require regular monitoring), you are able to request your blood test appointment using an admin form request. For patients who would like a blood test but have not yet discussed this request with a clinician, you will need to submit a medical request form.

However you choose to contact us, we may offer you a consultation:

  • by phone
  • face to face at the surgery
  • by text or email

Appointments by phone, text or email can be more flexible and often means you get help sooner.

Cancelling or changing an appointment

To cancel your appointment:

  • use your NHS account (through the NHS website or NHS App)
  • cancel online viat the link at the top of this page
  • phone us on 01825 732333 from 8am, Monday to Friday
  • reply CANCEL to your appointment reminder text message

If you have fallen over, or injured yourself in any way it may be more appropriate for you to go straight to one of the local Minor Injury Units, or Accident and Emergency Departments.

If you need help when we are closed

If you need medical help now, use NHS 111 online or Call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

If you need help with your appointment

Please tell us:

  • if there’s a specific doctor, nurse or other health professional you would prefer to respond
  • if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • if you need a chaperone
  • if you need an interpreter
  • if you have any other access or communication needs

Home visits

If you are housebound and need an appointment, we will do a home visit. We will phone you first to understand what you need.

To request a home visit, it’s helpful if you phone the practice as soon after 8am as possible.
